After completing and returning this form to a staff member or volunteer, please park your vehicle on the curb in front of the porch area if possible and wait on the porch for a staff member or volunteer to bring your items out to you. Due to COVID-19, only volunteers and staff are permitted inside the main Parish House building. Masks/face coverings are required on the porch. Please ask for a mask if you need one.
