This appears in a text about digging work on several tunnels, which form part of an extensive rail network in northern Spain:
Así, recientemente se han perforado dos túneles en los tramos Andoain-Urnieta y Urnieta-Hernani. La primera de las perforaciones tuvo lugar en el tramo que une las localidades de Urnieta y Hernani. Su longitud es de 5,2 kilómetros, y el recorrido discurre en su mayoría a través de túneles y viaductos. Para atravesar esta galería se ha utilizado una técnica diferente a la habitual, ya que se accedió a un punto intermedio del túnel y ***se inició la perforación hacia las bocas extremas***.

I think that what is meant here is that instead of using the usual method of starting at each end and meeting in the middle, in this case drilling began in the middle and went out to each end.
'Las bocas' in this case are just the openings at each end of the tunnel in my mind.
