If this meant "without the required expertise", then it should have been "sans l'expertise clinique requise". That makes me read it as a telegraphic form. Here are some examples of similar construction that seem to support my reading:

But, there is no "l'" before "expertise", which makes me read this as a telegraphic form of writing, i.e. omitting "aucune", which is considered "as read".

Discussion
Is this about prescription, in which case it may be about clinical expertise not being required?
Is this in reference to a requirement for an clinician's report at some stage?
I really don't know.
Expert report is fine.
This is pharmaceuticals. So that gives us a leaflet on pharmaceuticals being updated. I still don't get what the leaflet is actually about. About a drug? Is this to do with research or a clinical trial? Is it to do with adminisrtation of a specific drug?
What is the meaning of "changement classé "Cat"?
It's just that the term "expertise" can mean "expertise" in English (i.e. competence, skills, knowledge, capacity, etc.) but it can also mean "expert's report".
