Term appears in (and is the subject matter of) an Iberian Spanish medical PP presentation about healthcare processes for a specific disease, in this case acute coronary syndrome. From what I can make out, the term is increasingly common in Spain and is used to define an agreement made between healthcare professionals to establish a commonly accepted pattern or process for persons suffering from a particular condition.

Indeed, it refers to an established group of measures or 'blueprint' as a formalised standard response to a health issue. My suggestion for English would be 'protocol'. My etymological guess is a shortening for 'hoja de ruta' (map, plan of a trip, project, etc.).
