Estoy traduciendo el informe médico de una paciente anciana que fue hospitalizada por estar deshidratada y sufrir de anuria y tener septicemia de origen mixto, entre otras afecciones.

"Se colocó acceso venoso central subclavio derecho, se realiza reposición de líquidos y se inicia cobertura ATB con meropenen ajustado a función renal. Debido a la hipotensión profunda, que no respondía a líquidos, fue necesario apoyo vasoactivo, levophed por 24 h. Luego se logra mejoría de parámetros hemodinámicos y restitución de función renal (estuvo en anuria por 48 h). PCR de ingreso: 24. PCR de egreso: 10. Urocultivo reporta E. coli mulsensible. En control hematológico del día de hoy, se documenta nuevamente elevación de cuenta blanca con PCR estable (buena función renal y estabilidad hemodinámica) **por lo cual se plantea nuevo foco de piel**, celulitis sobreagregada a los edemas en miembros superiores, motivo por el cual se asocia vancomicina. "

Entiendo que la alta concentración de leucocitos indica una infección y el médico piensa que podría originarse en la piel.

¿Cómo se diría esto en inglés? Muchísimas gracias

Creo que se plantea en este contexto quiere decir se considera o se sospecha. Sospecha es lo más usual en el lenguaje médico. Ya que la cuenta blanca es elevada se sospecha un nuevo foco séptico en la piel.

En todo paciente con sospecha de sepsis se debe realizar el examen físico integral por órganos y sistemas para aproximarse al foco de origen.

Piel y partes blandas

En todo paciente con sospecha de sepsis de foco piel y partes blandas es imperativo tomar un GRAM y Cultivo de secreciones.

"Foco de piel" = skin infection. We can omit "nuevo" seeing as it is implied. If you look in the first link, you'll see "foco de piel y partes blandas" under "antibioterapia," the translation for which can be found in the second link.
